YOUR FIRST VISIT

Upon your arrival you will be greeted by Fallon, our schedule coordinator. During your first vist, a comprehensive clinical and radiographic evaluation will be completed by Dr Matthews and one of his assistants. The information gathered at this appointment is what will be used to help develop an assessment of your overall oral health and to help treatment plan any oral treatment needs. In order to help make this appointment successful, please bring with you a complete list of any prescription or over the counter medicines that you are taking, your completed new patient paperwork, your insurance card a photo ID.

OUR FINANCIAL POLICIES

Regarding Payment…
Full payment is due at the time of service. We accept cash, checks, VISA/MC, or Discover.
We also offer payment plans through Care Credit.

Regarding Missed Appointments…
In an effort to keep timely appointments our office does not “double book" appointment times. Our time is reserved and dedicated solely to you and you will seldom, if ever, have to wait. Because of this changes in our schedule can be catastrophic. We ask that any cancellations or changes to your scheduled appointment times be made at least 24 hours prior to your scheduled appointment. If we are not notified 24 hours prior to your scheduled appointment, then we may regrettably charge your account. Missed appointments are charged $40.

Regarding Contracted insurance…
Our practice participates with the following insurance plans: MetLife, Ameritas, Aetna, Blue Cross Blue Shield, and Delta. If you have any question whether or not our practice participates with your particular plan, please speak directly with the receptionist. If your plan is one with which we participate, we will bill and collect according to you plan. All deductibles, co-payments and disallowed charges will be due at the time of service.

Regarding Non-Contracted Insurance…
Your insurance coverage is a contract between you and your insurance company. Our professional services are rendered to you and not your insurance company. Therefore, you are directly responsible to us for the obligation of payment for treatment. We will do the utmost to help you derive the maximum benefits to which you are entitled, provided you supply us with current insurance card. Please be aware that some of the services provided may not be covered or may be considered above the “usual and customary.” Our practice is committed to the best treatment for our patients, while charging what is reasonable and customary for our area. You are responsible for your payment of your account, regardless of any insurance company’s arbitrary determination of usual and customary fees. If insurance has not responded to a claim within 60 days of submittal, the full account balance becomes the account holder’s responsibility.
